What to send

What to include with a referral.

A complete referral helps us verify benefits quickly and bill the visit correctly the first time.

The basics

  • The signed referral form, completed or simply signed
  • The patient's most recent progress note
  • Basic demographic info for the patient

Helpful to add

  • The diagnosis, with the ICD-10 code. We can only bill a medical diagnosis that is documented by the referring provider.
  • Your name and NPI. Some plans require the referring provider's NPI on the claim.
  • Recent labs, such as A1C, lipids or kidney labs, if you have them

Insurance and referrals

When a referral is required.

Most patients don't need a referral to see us, but some plans do.

Medicare

Medicare covers MNT only for diabetes, chronic kidney disease (stages 3 to 5, not on dialysis) and within 36 months of a kidney transplant. A referral from a physician (MD or DO), nurse practitioner or physician assistant that names the condition is required.

Medicare does not cover nutrition visits for weight or prediabetes. For patients on Medicare Part B or C, please ask them to call us first. Medicare dietitian coverage

Aetna

Aetna requires a referral or order for MNT. Please include the diagnosis so we can bill the visit correctly. Aetna dietitian coverage

HMO plans

HMO plans from any insurer require a referral on file before the first visit.

Most other commercial plans

Most other commercial plans cover nutrition counseling. We verify each patient's benefits before the first visit and tell them their cost in writing. Most insured patients pay $0 out of pocket. How insurance coverage works

Where patients are seen

In person in Alpharetta and Buckhead, or by video.

Emily Razza, MS, RDN, LDN, and Jackie Warren, RDN, LDN, see patients in person inside OptumMD's Alpharetta office (3542 Old Milton Pkwy, Alpharetta, GA 30005) and Buckhead office (91 W Wieuca Rd NE, Suite 1000, Atlanta, GA 30342). Melissa Logan, MHSc, RDN, LDN, sees patients by video. Meet our dietitians.

With virtual visits, we can see clients in Arizona, California, Colorado, Florida, Georgia, Pennsylvania, Virginia and Washington. The patient needs to be in one of these states at the time of the visit. We bill insurance in network for clients in Georgia, Florida, California and Pennsylvania.

Our main office is in Roswell (675 Mansell Rd, Suite 115). In-person visits are not offered there for now.

After the first visit

The dietitian sets a follow-up schedule at the first visit. Most clients who do well meet weekly or every other week for about their first 8 visits. We keep you informed of your patient's progress along the way.

Questions

Questions from physician offices.

What should we send with a referral?

Complete our referral form, or simply sign it and send it with the patient's progress note and basic demographic info. Fax it to 888-356-0405. Including the diagnosis and ICD-10 code helps us bill the visit correctly.

Does my patient need a referral to see a dietitian?

Not always. Medicare, Aetna and HMO plans require one. Many other commercial plans do not. We verify each patient's benefits before the first visit and let them know if a referral is needed.

Does Medicare cover medical nutrition therapy?

Yes, for some conditions. Medicare covers MNT only for diabetes, chronic kidney disease (stages 3 to 5, not on dialysis) and within 36 months of a kidney transplant, and it requires a referral from a physician (MD or DO), nurse practitioner or physician assistant. It does not cover nutrition visits for weight or prediabetes.

How quickly will my patient be contacted?

Once we receive your referral, we verify the patient's nutrition benefits and contact the patient to schedule. Patients can also book online themselves at app.web-rd.com.

How will we hear about the patient's progress?

We keep the referring provider informed of the patient's progress. Questions about a patient? Call or text 470-731-9176.

Do your dietitians prescribe or adjust GLP-1 medication?

No. Our dietitians never prescribe or dose medication. We provide nutrition support alongside the prescriber, who manages the medication.

Ready to refer a patient?
